php and html code can be separately written and can mix up easily. take an example of wordpress - wordpress application has been written in php and the themes of wordpress are mostly design through css and html. you can easily add any theme into wordpress to change the look. if your wordpress theme look blue, just change some css stuff and make it green .
for php coders, developing using both - php and html would affect the efficiency, rather its better to assign the designing stuff to the people who are good at html.

The only thing i know about smarty is that it is a web template system written in PHP. Smarty is primarily promoted as a tool for separation of concerns, which is a common design strategy for certain kinds of applications.

or it may be the templating engine used by CiviCRM.
